PHASE ONE, INDEPENDENT LIVING PROGRAM PLUS

The Independent Living Program Plus (ILP) program provides a structured therapeutic living environment with 18 hour supervision. Case managers and on call staff are accessible during the time youth are unsupervised. Youth advocates document daily life skills progress and acitivites of the youth. Youth are assigned daily chores and follow a daily agenda. Youth advocates are positive role models and are responsible for guiding the youth towards success by providing advocacy, life skills training, supportive services, supervision and crisis intervention in a professional manner. All life skills trainings are coordinated by the life skills coach. Youth must remain in phase one for at least nine months. They youth must meet the following criteria before advancing to phase teo: land lord/tenant training, gainfully employed, meet life skills requirements according to treatment tram standards, display self-determination and is compliant with goals and objectives set fourth in the person centered service plan.

PHASE TWO, GENERAL INDEPENDENT LIVING

Once a youth has successfully completed the ILP componenet of the program they are eligible for phase two. Phase two is a less restrictive living arrangement that last for 3 months. During this time, the youtn must maintain their living environment,pay bills, work and or attend school, manage money, food and stay on budget. The social service worker checks on the youth at least three times a week to monitor the youth’s progress. Each youth will enter into a contractual agreement regarding their obligations and expectations. Youth that don’t successfully complete phase two may be dropped back down to phase one. Youth are placed in apartments/houses contracted by the agency. A stipend will be provided for the monthly rent and living expenses. Youth must have at least $1200 saved up to meve to phase three.

PHASE THREE, SUPPORTIVE HOUSING

The supportive housing program provides a one-time security deposit, first-month rent, and start up goods as well as case management services. The youth must successfully complete phase two and meet the ILP requirements which include landlord/tenant training, successful completion of independent living skills training goals, gainfully employed, and savings account with a balance of a minimum of $1200. CCO will provide case management services to youth in the supportive housing program until they age out of the system. The social service worker meets with the youth monthly at their residence to ensure that the youth is maintaining their environment. Youth that turn 18 while in the program must be continuing their education in order to remain in the program.
