3rd Shift Cleanliness Report

When Youth Advocates clocks-in, a thorough walk through of the facility must be completed. Everything that the Youth Advocates can see is wrong with the housing facility must be documented and addressed during Youth Advocate’s shift. First shift will arrive at 2:45 pm to conduct first walk through. Midnight shift will arrive at 10:45 pm to conduct second walk through. Youth Advocate (s) will select either Clean (C) or Dirty (D) for condition upon arrival or departure. Followed by comments or maintenance emergencies on the condition of the facility.
