This form must be complete every sheet. Youth advocates are required to do a detailed walk through of the facility upon arrival. The goal is to identify any maintenance issues that need to be addressed.

Run water to make sure it's running, Flush toilet to make sure its not clogged.

Inspect Window Screens, do they need to be replaced?

Inspect Ceiling for Leaks, and/or damage
